Verdun Home Health Care Services is a New York Licensed Home Health Agency that is Joint Commission Accredited. We have been tested, evaluated and earned The Joint Commission’s Gold Seal of Approval. As a full service Agency, we are dedicated to the delivery of superior service to all our clients and are driven by that work ethic.

We strive to offer compassionate, quality home health care to individuals and families.

    Ready-Set-Go Care from the hospital to home.
    We'll be there all the way.
    Ready-Set-Go Home is a transitional care program that is designed to help seniors make a successful transition home after hospitalization. This program includes a three-prong approach for reducing hospital readmissions: a major risk for seniors in the month after a hospital discharge.
